FAITH TALK Video Release Form

Welcome and thank you for engaging in our Prince of Peace "Faith Talk" ministry project by sharing a one minute/100 word or less video of your journey. For us to proceed, we will require you to fill out this release form, granting Prince of Peace permission to use your submitted video(s) in our communications.
If you are submitting a video including a minor child/children you will need to complete sections A & B as shown, otherwise, skip section B if not relevant.
Upon providing your full name, thus indicating your electronic signature, and sending in this form, you will receive a confirmation email with instructions on how to share/upload your video for Prince of Peace to use.
For best results, please view Trevor's instruction video for help in making a Faith Talk video if you have not done so already. (Refer back to the Faith Talk page on www.popappleton.org.) If you have any other questions about making or sending in a Faith Talk video, please email us at popfaithtalks@gmail.com or contact the church office (739-5255). By electronically signing and submitting this consent/release form, I grant permission to Prince of Peace Lutheran Church, Appleton and it’s representatives to record, reproduce and/or publish my photo/video/likeness for the purpose of church communications using multi-media platforms (such as but not limited to the projection, website, email campaigns, social media postings, and/or by linking to other digitally shared church-related communications.

Section B -

By submitting the names of my minor child/children below, I grant permission to Prince of Peace Lutheran Church, Appleton and it’s representatives to record, reproduce and/or publish the listed child/children's photo/video/likeness for the purpose of church communications using multi-media platforms (such as but not limited to the projection, website, email campaigns, social media postings, and/or by linking to other digitally shared church-related communications. I understand my family's adult first and/or last name may be published in association with released photo/videos, but will not publish the name(s) of minor child/children.
